One of the requirements of eligibility for FMLA for a family member is to provide sufficient documentation to establish a relationship between the employee and the family member. How have you handled those requirements when the family member – mother – has been married multiple times and may not have the documentation (marriage certificate, divorce decree)? The mother’s name on the employees birth certificate (maiden) doesn’t match her current last name.
